Botox Injections for Migraines – Worth the Trouble and Expense?

ARE BOTOX INJECTIONS WORTH THE TROUBLE AND EXPENSE FOR PREVENTING CHRONIC MIGRAINE HEADACHES?

A recent study published in the April 25th issue of the Journal of the American Medical Association (JAMA) reviewed the results of 31 published studies on the use of Botox injections to prevent migraine headaches.

The researchers found that the use of botox injections to prevent occasional migraines (episodic migraines defined as less than 15 headache days per month) and chronic daily headaches was of no greater benefit than placebo.

They further reported that for chronic migraines (15 or more migraine or headache days a month) or chronic daily headaches, there was a small to modest benefit that was probably not clinically very significant. In addition, compared to placebo, Botox injections caused:

  • Neck discomfort or pain
  • Muscle weakness
  • Paresthesias (a prickly, tingling, electric shock type sensation)
  • Drooping of the upper eyelid
  • Skin tightness or discomfort

The bottom line is that for most people, based upon this analysis of 31 published studies, the expense and effort of going through Botox injections to try to prevent chronic migraine headaches may not lead to satisfactory results.

Vestibular Disorder or Migraine with Vestibular Symptoms?

The term “vestibular migraine” is not a real medical classification. A more accurate description would be a migraine with vestibular symptoms.

The vestibular system in the inner ear, is one that maintains balance and equilibrium. Therefore vestibular symptoms are dizziness, vertigo (a sense of spinning or motion when at rest), or loss of balance and disequilibrium.

Basilar migraines can also present with vertigo and tinnitus. Menieres disease (a condition with similar symptoms) is often diagnosed when in fact the patient my be experiencing migraines with symptoms of vestibular disorder. It is known that people with migraines are more apt to experience Menieres and vice versa.

Patients with migraines with these symptoms can experience just vestibular symptoms or the symptoms along with migraine head pain. The MRI brain scans of these patients are usually normal.

Though there have been NO control trials demonstrating efficacy for migraines with vestibular symptoms, the following drugs are used by physicians. For frequent or chronic vestibular migraines, daily calcium channel blockers (a blood pressure medication), SSRIs (a type of anti-depressant), and anti epileptic drugs. Also triptan drugs are used for acute attacks but once again there are no trials showing that they help with the vestibular symptoms.

Though there have been organized clinical trials demonstrating the prophylactic efficacy of high-dose riboflavin and magnesium for treating chronic migraines, there are NO trials showing these ingredients, either alone or in combination reduce or eliminate vestibular symptoms.

Many chronic migraine patients with the symptoms of vestibular disorder who were using MigreLief, report a decrease in these symptoms and an increase in the time they felt their balance and equilibrium were closer to normal.

Teen migraine vs. adult migraine

Migraines are extremely painful and often debilitating for teens and children. Just like adults, teens can also experience dizziness, vertigo, nausea, sensitivity to light and noise, a lack of energy, disturbed vision, and other symptoms. Teen migraines are often different than adult migraines. While adult migraines often last several hours or more and settle on one side of the head, in a child or young teen, a migraine may last for as little as one hour and the pain is often felt across the front of the forehead or on both temples.

As a result, childhood & teen migraines are often dismissed as sinus headaches.  Some pediatric/teen migraines don’t involve headache pain at all.  Instead, the child or teen may have abdominal pain, vomiting, or feelings of vertigo.  The attacks may be shorter, but after the migraine disappears it can cause an ill feeling that can be even worse than the head pain. Of course, symptoms can vary from person to person and can also vary from one attack to the next. Teens, like adults, and sometimes even more so, can be severely affected by stress. Incorporating stress reduction activities like deep breathing and meditation can be helpful in lessening migraine attacks.

  • Hormone changes such as just before the menstrual period begins

“Parents should be aware that our medication choices aren’t as good as they should be,” Dr. Jennifer Bickel, a neurologist and headache specialist at  Children’s Mercy Hospitals and Clinics in Kansas City, Missouri, told Genevra Pittman at Reuters Health.

There are not many medications for treating children’s migraines that are without major controversy, especially the class of medication known as Triptans. If you do your own research, you will see in journals where triptans (such as Children’s Imitrex) are being considered or even approved for children by the FDA, but you will find as many or more articles saying that triptans are in no way safe for children. The only medicine deemed “safe” for treating children’s migraines is ibuprofen. Anyone who has suffered a migraine will tell you that ibuprofen doesn’t begin to mitigate the pain, nor will it prevent future migraines.

Topamax (topiramate), a medication to treat children’s migraines prophylactically (preventively) is an anticonvulsant used to control epilepsy and has also been promoted as a weight-loss drug. This same drug has been shown in clinical trials to cause a full 27% of child test subjects to lose weight. Other side effects are known to be an upper respiratory infection, anorexia, and paresthesia. These were the results of the only major clinical study conducted on topiramate for children. Outside of the general discussion of Topamax’s side effects, one source (http://epilepsy.emedtv.com) cited that side effects such as memory loss or difficulty concentrating make it difficult to tolerate Topamax. Such side effects could be devastating for a school student, especially if the child already suffers from ADHD.

Additionally, if the child is already being treated pharmacologically for ADHD, depression, anxiety, stress, anorexia, or anything requiring medication, adding yet another medication to those already stressing the child’s body is simply unsound at best, and life-threatening at worst. Combining other medications, especially antidepressants, with migraine medications such as those mentioned above can raise serotonin levels too much and result in serotonin syndrome. 

According to an article in the New England Journal of Medicine, “Serotonin syndrome is a potentially life-threatening adverse drug reaction that may occur following therapeutic drug use, or as an inadvertent interaction between drugs.” Even a commonly used over-the-counter cough suppressant ingredient, dextromethorphan, combined with a migraine medication can cause serotonin syndrome.

The Migraine and Weight Gain Connection?

A recent study carried out by the University of Washington in Seattle and published in the medical journal “Headache” found that women whose migraines started in childhood were 33% more likely to gain 22 pounds or more after the age of 18 than women who didn’t have migraines.

It is known that obesity doubles the odds of getting migraines and that over-weight children who were suffering from migraines, improved significantly, if they lost weight.

With a focus on nutritional applications of herbal and natural medicine (based on scientific application) Curt’s unique combination of education, training, and experience in chemistry, medicine, nutrition, the supplement industry, NIH sponsored research, and pharmaceutical drug development, generates a perspective leading to an intimate understanding of the workings of the human body.  After spending two years in a physical chemistry Ph.D. program at the City University of New York, Curt obtained a Master’s Degree in Human Nutrition from the University of Bridgeport. He is board-certified both in clinical nutrition (CCN) and as a nutrition specialist (CNS) from the American Academy of Nutrition.

Curt has spent the last several decades researching and archiving the potential benefits, effects and mechanisms of literally thousands of naturally occurring compounds (plants, amino acids, vitamins/minerals and their metabolites and other nutrients and supplements).  His efforts in developing “natural-based” medicines led to his creating disease-specific monographs in areas such as Migraine, Osteoporosis, Osteoarthritis, Benign Prostatic Hyperplasia, Depression and more, approved for Continuing Education credits for physician license renewal.

His extensive research, knowledge and expertise have also led to the formulation and manufacturing of various condition-specific nutritional supplements.  Some of these supplements and drugs are currently available to the public, others are pending release. He holds many U.S. patents with additional patents pending.

Researching and developing safer natural medicines and leading-edge solutions to combat disease keep Curt busy but he still finds time to travel throughout the United States, sharing the science behind the success of  MigreLief’s line of nutritional supplements for migraine sufferers.


CREATING MIGRELIEF – a personal note:

I understand the impact that migraines have on the life of a sufferer, their friends and loved ones.  For the past 27 years, I lived with the pain, worry and fear that migraine attacks bring on. Over 25 Million Suffer with Chronic Daily Pain

Over 25 Million People in the U.S. Suffer with Chronic Daily Pain. Is there a Safe Alternative to the Chronic Ingestion of Pain Killers and their Associated Side-Effects?

What can the 25 million people in the United States who suffer chronic daily pain do that is safe and won’t expose them to the health risks of chronically ingesting pain killersSide-effects include heart attack, stroke, kidney damage, liver damage, ulcers and bleeding.

Over 20 years ago, due to playing competitive tennis on the cement courts of California 3-4 times a week, my knees deteriorated and even though I was in very good shape, my daily pain was so severe,  just stepping off a curb to cross the street was a very painful challenge.

I decided to see  a famous orthopedic knee surgeon, who took care of and operated on the knees of some of the most high paid and well known athletes in professional sports.

I didn’t tell him I was a chemist and knew a lot about pharmaceutical drugs and pathway’s of pain and inflammation throughout the body.  I was there to keep quiet and listen to what he had to say and what he recommended.

After doing a very thorough exam, and taking all of the necessary “pictures” of my knees, he told me to come back in a few days to discuss his recommendations.

Upon my return, I was told (I paraphrase in non-medical terms) that my knees were about as bad as he had ever seen, and that I should have had bilateral knee replacement years ago.  I asked  “point blank” if there was any chance that the pain would go away on its own and if there were non-surgical options to reduce or eliminate the pain?  His comment was, “You don’t have a 1% chance of alleviating your pain without the knee replacement.”

I thanked him, paid his fee and upon leaving, knew that I would not be following his advice until I exhausted every “non-surgical” option I could uncover that had both reasonable scientific support or logic and was safe.

Honestly, 20 years ago, though I was quite knowledgeable about body chemistry and natural medicines, I know much more now then I did then, but fortunately I made some “educated” lucky guesses about what to take!

Fast forward 20 years –  As I sit here writing this article, I can say that I haven’t had knee pain for the last 18 years and I never had the knee replacement that the “expert” unequivocally said was “my only chance.”

I can walk for miles with absolutely no pain.  Radiographically, I doubt (but I haven’t checked) that the bone on bone status of my knees (lack of cartilage cushioning the knee joint) has changed dramatically (if at all) but my pain is gone!

If I jam my knee on a hard surface from running or jumping to aggressively, I will feel some pain or discomfort (so I try not to do that).  But for all practical intents and purposes, my knees are “Pain Free.”

I tell you this story because of an article I recently read which analyzed data from the 2012 National Health Interview Survey (NHIS) which was carried out by the National Institutes of Health  National Center for Complementary and Integrative Health (NCCIH).  The analysis revealed that over 25 million adults suffer with daily pain (14.4 million of whom rated their pain at the most severe level).

This does not include those people who suffer from chronic pain that isn’t experienced on a daily basis.

The question that still rises to the top of my mind today, just as it did over 20 years ago, “Other than undergoing surgery or taking a daily synthetic drug, all of which are known to have specific kinds of health and side-effect risks…….What can I do?”

This is the same point that was posed by the NCCIH researchers and authors of the study.  They stated:  “Pain may not be adequately controlled with prescription drugs or other conventional treatments… A research priority of the NCCIH is to determine the effectiveness of complementary approaches for treating pain.”

A review of the published medical literature reveals that the health risks caused by chronic ingestion of either OTC or Rx Nonsteroidal Anti-inflammatory Drugs (NSAIDs) like aspirin or ibuprofen or naproxen or pain killers like acetaminophen range from: gastrointestinal bleeding, ulcers, stoke, cardiovascular issues, kidney damage and liver damage depending upon which drug is chronically ingested.

Are NSAIDs safe?

 

Though the risks have been known for quite a while, lately more and more high-profile institutions have been writing about them. The FDA issued a warning in 2014 basically saying:  Stop dispensing and stop taking prescription medications containing more than 325 milligrams of acetaminophen. It is  now the second most common cause of liver failure requiring transplantation in the United States.

Because acetaminophen is available over the counter, many consumers don’t realize that it may also be in prescription drugs they get from their physicians and dentists and also after hospital procedures.

Very recently the FDA strengthened its warning about the increased risk of heart attack and stroke caused by  NSAIDs like ibuprofen  and naproxen. (aspirin, also an NSAID, was not associated with these risks but can cause spontaneous gastrointestinal bleeding and stomach disorders like ulcers.)

I was aware of these issues at the time of my severe knee pain, so none of these were ever an option that I would have considered.

By this time many of you may be thinking – “OK, I got it. I won’t take those things or I want to stop…  What did you do?”

I identified specific kinds of herbal extracts such as: ginger, boswellia and feverfew and took them daily at very specific doses. To these ingredients I added the simple mineral magnesium which was known amongst many other health benefits, to block a receptor known to abet the pain response.

Over the years I also added a turmeric extract (curcumin) and a high quality omega-3 fish oil delivering at least 1500 mg/day of combined  EPA and DHA (eicosapentanoic acid and docosahexanoic acid).  My pain however, started to significantly decrease just using the first 4 ingredients mentioned above (Ginger, Boswellia, Feverfew and Magnesium)

I believe the two mechanisms of action that make this daily regimen so effective against all kinds of pain is that the components have been shown both to block multiple factors contributing to excessive/chronic systemic inflammatory response and the creation of excessive levels of certain types of reactive oxygen species both of which are intricately involved in causing tissue damage and body pain.
